At Community Primary Care, our mission is to provide our patients with high-quality, comprehensive, patient-centered medical care with the goal of optimizing our patients’ overall well-being.
That has been our mission since 1997 when Rabi R. Sinha, MD began as a solo practitioner in Hopewell Junction before attracting other providers who believed in this same philosophy. That’s when Community Primary Care was born.
Today, our team of six providers and additional support staff serve the Hudson Valley Community and are committed to delivering first-class, primary care services. We practice medicine with an integrated approach to patient care, coordinating with subspecialists as needed and using a state-of-the-art electronic medical record system for seamless care.
We strive to offer everyone the access and control they need over their healthcare experience in a warm and friendly environment.
Community Primary Care is participating in a pilot study of Medicare known as Comprehensive Primary Care Initiative(CPCI). We are 1 of 75 practices chosen here in New York. This government initiative aims to foster collaboration among certain public and private health plans to strengthen primary care.
Community Primary Care has been designated a Patient Centered Medial Home with level 3 accreditation by the National Commitee for Quality Assurance (NCQA).
Our office is located in a state-of-the-art medical complex, conveniently located just minutes from the Taconic State Parkway. Our facility is wheelchair accessible with adequate parking.
Community Primary Care is affiliated with Saint Francis Hospital, Vassar Brothers Medical Center and Putnam Hospital Center.
